You have to use the D-mic when emma is walking across those pipes. It happens when she is behind one of he buildings I think.

i found what i was talking about yesterday.

Mental exertion also plays its part; there's a running gag in the game that sees a quarter of the pshche guage wiped out everytiume a character makes a jibe over his advancing years.
The psyche guage is linked to the stress guage. Spend too long in prolonged combat with concentration fading and Snake's stress level will rocket, hit 100% and ther's a short term benefit - a 'combat-high' that gives him superb accuracy and accelerated healing. However snake will then 'crash', which leaves him temporarily weaker than ever. Not good when the Gekkos are incoming...
